2. Eligibility

You must be 18 years or older to submit information. By doing so, you confirm that you are the legal decision-maker or are acting on behalf of someone in need of care.

3. Communication Consent

By submitting your information, you agree to receive:

SMS, phone, or email communication from Nurse Next Door HamiltonAppointment reminders and service-related follow-ups

You may opt out of messaging by replying “STOP.” We comply with
